【 VX 是什麼？】
✍ VX 是 venomous agent X 的縮寫，是一種有機磷神經毒素 (organophosphate nerve agent)，純品為無色無味、低揮發性、高黏滯度的油狀液體。VX 具有高穩定性、低揮發性的物化性質，使 VX 即便在爆炸性分散後依然能有高度且持久的毒性，可以說是恐怖攻擊生化武器之首。
　　　
【 VX 的機轉是？】
✍ VX 為抑制乙醯膽鹼酯酶 (AChE)，導致乙醯膽鹼無法被水解而過度累積。中毒路徑包括吸入、食入和皮膚接觸。初期症狀是接觸部位肌肉抽搐或大量排汗，接著噁心及嘔吐；吸入接觸者，則會出現流鼻水、劇烈支氣管收縮和胸悶；縮瞳 (miosis) 可以說是最明顯的早期症狀（所以當下狂點散瞳劑 atropine 可能就... 就死的比較有尊嚴一點），而後期則會導致中樞抑制、癲癇、心律不整、全身抽搐，最終呼吸衰竭而死。
　
【 VX 有比含笑半步癲還毒嗎？】
✍ VX 這類神經毒氣可以說是現今最強生化戰劑，只要短暫接觸即可能在 1 - 10 分鐘之內死亡，若暴露濃度較低則可能延遲到 1 - 2 小時才死亡。
。VX 氣化後的毒性，是著名的沙林毒氣 (Sarin) 的 100 倍！
。VX 皮膚接觸的毒性，是著名的水果香化學武器－塔崩 (Tabun) 的 300 倍！
👑 因此，我們認為 VX 的毒發速度快過一日喪命散，毒性劇烈狂過含笑半步癲。
　　
【 VX 的治療】
✍ 建議使用有機磷之卍解解毒套餐：atropine + pralidoxime (2-PAM)。
。Atropine: antagonize muscarinic effects
。2-PAM: reverse neuromuscular blockade
✍ 美國有軍用 autoinjector （現役美規：ATNAA - Antidote Treatment Nerve Agent Autoinjector），每一套 kit 含有兩套自動注射針，分別為 atropine 2.1 mg/0.7 ml 與 2-PAM 600 mg/2 ml，直接肌肉注射即可（或許不需要像凱吉哥一樣捅心臟？）。
　
補充：ATNAA: DOSE: ADULT: One ATNAA into the lateral thigh muscle or buttocks. Wait 10 - 15 min for effect (Prod Info ATNAA ANTIDOTE TREATMENT – NERVE AGENT, AUTO-INJECTOR intramuscular injection solution, 2002).
-> 建議 IM 戳大腿肌或臀大肌

補充：Therapeutic plasma concentrations of pralidoxime exceeding 4 mcg/mL were achieved within 4 - 8 min after injection (Sidell & Groff, 1974).

✍ 若癲癇發作時，可使用 diazepine 等 BZDs，並考慮合併 phenobarbital, propofol 等其他藥品協同治療。

自律神經系統 (autonomic nerve system) 包括交感 (sympathetic) 與副交感 (para-sympathetic) 神經，可以說是掌控超多器官、組織的重要神經。

神經傳導要轉化成化學訊號，需要神經傳導物質 (neurotransmitter) 的幫忙，在交感神經就是NE (norepinephrine 正腎上腺素)，在副交感神經就是ACh (acetylcholine 乙醯膽鹼) 了。

★★合成★★
在副交感神經突觸前終端，Acetyl-CoA + choline = Acetylcholine，而交感神經，是tyrosine (酪胺酸) 經酵素作用為L-Dopa，再變成Dopamine (多巴胺)，最後是Norepinephrine (正腎上腺素)。

★★作用★★
一旦釋放到突觸間隙，ACh作用在突觸後膽鹼激性受體、而NE作用在腎上腺受體

★★分解★★
ACh在突觸間隙被AChE (acetylcholinesterase) 分解為choline、acetate；而NE被COMT (catechol-O-methyl transferase) 分解為代謝物，繼續生生不息。

★★特點★★
請注意，交感神經突觸前終端的alpha-2腎上腺受體，是一種自我調節 (auto-regulation) 受體，就像油門跟剎車一樣，NE除了作用在突觸後的腎上腺受體外，也可以透過這個受體調節NE的釋放。

老師雖然每次在提供單字表時都會附上中文解釋，但這只是方便學生更快知道這個單字最通用的意思，不代表是它最完整的涵義及用法。除此之外，有心的同學應該進一步使用其它工具查詢，最好是單字，配詞與句子一起背 (死背一個單字是無用的)，再透過經常使用單字的方式，加強自己對單字的認知和應用。當然，你也可以利用老師或字典提供的單字音檔，進而增加自己對單字的辨識能力和發音!
