"หรือเราต้องฉีดวัคซีนเยอะเกิน 100% ? ถึงจะรับมือโควิด สายพันธุ์เดลต้าได้"

ต่อเนื่องจากประเด็นที่มีการพูดว่า ประเทศไทยเรา วางแผนจะฉีดวัคซีนโรค COVID-19 ให้ได้ 50 ล้านคน เพื่อให้เกิดภูมิคุ้มกันหมู่ (herd immunity) หยุดการแพร่ระบาดได้

แต่ก็เกิดการโต้แย้งตามมาว่า มันเป็นตัวเลขที่เพียงพอจริงหรือเปล่า ? โดยเฉพาะในสถานการณ์ที่ประเทศไทยเรา มีการแพร่ระบาดของไวรัสสายพันธุ์เดลต้า (delta) และใช้วัคซีนที่ประสิทธิภาพไม่ได้สูงมากด้วย

เลยไปลองหาบทความ ที่เกี่ยวกับการที่ "นักระบาดวิทยา" ในประเทศสหรัฐอเมริกา เขาวิเคราะห์ว่าจะต้องฉีดวัคซีนเท่าไหร่ ถึงจะรับมือกับสายพันธุ์เดลต้าได้.. เผื่อจะมาเทียบกับประเทศไทยเราครับ

1. ศูนย์ CDC (The Centers for Disease Control and Prevention) ของสหรัฐอเมริกาเปิดเผยข้อมูล "เบื้องต้น" ว่า เชื้อไวรัสโควิดสายพันธุ์เดลต้านั้น อาจจะมีค่า basic reproductive number (R0) อยู่ระหว่าง 6 ถึง 9

- ค่า R0 เป็นตัวเลขทางระบาดวิทยา แสดงถึงจำนวนคนโดยเฉลี่ยที่ติดโรค ได้จากผู้ติดเชื้อ 1 คน ถ้าประชากรนั้นไม่มีภูมิคุ้มกันกับเชื้อโรคเลย

- ค่า R0 ของโรคหวัดตามฤดูกาล และไข้หวัดใหญ่ มีค่าระหว่าง 1 ถึง 2 ขณะที่ค่า R0 ของโรคอีสุกอีใส มีค่าระหว่าง 12 ถึง 18

2. แต่จริงๆ แล้ว ในทางระบาดวิทยานั้น มีอีกค่าหนึ่งที่สำคัญจะต้องคิด คือ ค่า effective reproductive number หรือ Re (pronounced arr-ee)

- ค่า Re นั้น เปรียบเสมือนค่า R0 ที่เอาไปใช้จริงในสถานการณ์จริงของโลก (real-world scenario) โดย Re จะเป็นตัวเลขที่บอกถึงอัตราการระบาดของโรค เมื่อมีปัจจัยอื่นๆ เข้ามาเกี่ยวข้องด้วย เช่น การที่ประชาชนใส่หน้ากากอนามัย การเว้นระยะห่างทางสังคม และการฉีดวัคซีน ทำให้ค่า Re ไม่ได้เป็นค่าคงที่แบบค่า R0 แต่จะเปลี่ยนไปตามเวลาและสถานที่ ที่มีระดับภูมิคุ้มกันและมาตรการควบคุมแตกต่างกันไป

- เราประมาณค่า Re ได้จากการเอาค่า R0 มาคูณด้วยอัตราส่วนของประชากร ที่อ่อนแอต่อการเป็นโรคนั้น (หรือคูณด้วย "1" ลบอัตราส่วนของประชากรที่ได้รับวัคซีนเรียบร้อยแล้ว) ตามสมการ Re = R0(1-x*v) เมื่อ X เป็นเปอร์เซ็นต์ของประชากรที่ได้รับวัคซีนครบโดสแล้ว และ v ประสิทธิภาพของวัคซีน

3. เป้าหมายของการควบคุมโรค ไม่ใช่เรื่องการทำให้ผู้ติดเชื้อกลายเป็น 0 คน แต่จะพยายามทำให้ค่า Re เท่ากับ 1 เพราะถ้ามีค่ามากกว่า 1 ก็แสดงว่าโรคกําลังระบาด ขยายตัวออก จะมีผู้ป่วยมากขึ้น

- ถ้าเชื้อไวรัส COVID-19 สายพันธุ์ดั้งเดิม มีค่า R0 ประมาณ 3 และวีคซีน mRNA (เช่น ของ บ. ไฟเซอร์ และ บ. โมเดิร์นน่า) มีประสิทธิภาพในการป้องกันการป่วยหนักและเสียชีวิตประมาณ 95%

- ถ้าสามารถฉีดวัคซีนนี้ได้ครบทุกคน (ในสหรัฐอเมริกา) ค่า Re สำหรับสายพันธุ์ดั้งเดิม จะมีค่า = 3*0.05 = 0.15 ซึ่งค่าต่ำมากๆ ดีมากๆ

- แต่ข้อมูลเมื่อวันที่ 2 สิงหาคม ที่ผ่านมา มีประชาชนชาวสหรัฐอเมริกาประมาณ 58% ที่ได้รับวัคซีนแล้วอย่างน้อย 1 โดส และประมาณ 50% ของประชากรที่ได้รับวัคซีนครบโดส

- ขณะที่วัคซีนเอง ก็ไม่ได้เพอร์เฟค นั่นคือ ประมาณ 47% คนอเมริกันที่ได้รับการปกป้องด้วยวัคซีน จะทำให้ค่า Re ที่คำนวณได้อยู่ที่ประมาณ 1.6 (ซึ่งมีค่าสูงกว่า 1) และแสดงว่ายังมีการแพร่ระบาดเกิดขึ้นได้ แต่สถานการณ์ก็ดีขึ้นเยอะ (ทำให้รัฐบาลประกาศให้ถอดหน้ากากได้)

- สมมุติกำหนดให้ค่า R0 = 8 แล้ววัคซีน mRNA มีประสิทธิภาพลงมาที่ประมาณ 85% (คือมีประชากรที่อ่อนแอต่อการเป็นโรคอยู่ 53%) ค่า Re ต่อสายพันธุ์เดลต้า จะมีค่าสูงถึง "5" แม้ว่าจะฉีดวัคซีนไปครบโดส ถึงครึ่งนึงของประชากรแล้วก็ตาม !

- ดังนั้น ประเทศสหรัฐอเมริกาจำเป็นจะต้องฉีดวัคซีนกับประชากรให้ได้เพิ่มมากกว่านี้ เช่น ถ้าฉีดได้ถึง 75% ค่า Re จะลดลงมาที่ต่ำกว่า "3" เป็นระดับที่มาตรการทางสาธารณสุข เช่น การใส่หน้ากากอนามัย ไปจนถึงการหยุดดำเนินงานของธุรกิจต่างๆ จะเพียงพอที่ควบคุมการระบาดได้

5. สำหรับกรณีของประเทศไทยเรา ซึ่งมีการใช้วัคซีนประสิทธิภาพปานกลางอยู่แล้ว อย่างกลุ่มวัคซีนแบบไวรัลเวกเตอร์ (วัคซีน astrazeneca) และวัคซีนเชื้อตาย (วัคซีน sinovac และ sinopharm)

- วัคซีนเหล่านี้ก็จะมีประสิทธิภาพลดต่ำลงเช่นกัน เมื่อเจอกับสายพันธุ์เดลต้า

- คำถามที่ฝากไปสำหรับ "คนที่ชอบคำนวณ" ก็คืออย่างนี้เราจะต้องฉีดวัคซีนให้ได้ครบโดส เป็นจำนวนกี่เปอร์เซ็นต์ของประชากร ถึงจะทำให้ค่า Re ลงมาเหลือ 3 ได้ จากที่เคย Re เท่ากับ 8 สำหรับสายพันธุ์เดลต้า
