โรคพร่องเอนไซม์จีซิกซ์พีดี (G6PD)คืออะไร
บอกไว้ก่อนเลยครับ โรคนี้ผมก็เป็น(คุณจะไม่เป็นโรคนี้อย่างเดียวดาย 555) You'll never walk alone เรามาทำความเข้าใจไปพร้อมๆกันนะครับ โรคพร่องเอนไซม์จีซิกซ์พีดี (G6PD)หรือ Glucose-6-phosphate dehydrogenase deficiency พร่องก็คือมีเอนไซม์ตัวนี้น้อยกว่าคนอื่น เป็นโรคทางพันธุกรรมโรคหนึ่ง(เกิดมาก็เป็นเลย กรรมจริงๆ) คนที่เป็นโรคนี้เม็ดเลือดแดงแตกจะง่ายเมื่อได้รับสิ่งกระตุ้นต่างๆ
ขอเล่ากระบวนการอันแสนซับซ้อนทางวิทยาศาสตร์ให้เป็นภาษพูดนะครับ
จีซิกซ์พีดี เป็นเอนไซม์สำคัญในกระบวนการย่อย(metabolism)น้ำตาลกลูโคส ทำให้ได้สารตัวนึงชื่อ NADPH ซึ่งสาร NADPH เป็นสารสำคัญส่วนหนึ่งของกระบวนการทำลายสารอนุมูลอิสระต่างๆ เช่น ไฮโดรเจนเปอร์ออกไซด์ ที่เป็นพิษต่อเม็ดเลือดแดง
ดังนั้นเอนไซม์จีซิกซ์พีดี จึงช่วยป้องกันเม็ดเลือดแดงจากการทำลายของสารอนุมูลอิสระ ใครที่ขาดเอนไซม์ตัวนี้อาจจะทำให้เม็ดเลือดแดงแตกได้ง่าย สาเหตุของการพร่องเกิดจากความผิดปกติของพันธุกรรม(แบบ X-linked recessive) โรคนี้จึงเกิดอาการในผู้ชายได้มากกว่าผู้หญิง
ผู้ป่วยมักไม่มีอาการ แต่เมื่อได้รับสิ่งกระตุ้นต่างๆ เช่น ถั่วปากอ้า หรือ ยาบางชนิด จะเกิดภาวะซีดจากการที่เม็ดเลือดแดงแตกอย่างเฉียบพลัน เหนื่อยง่าย อ่อนเพลีย ตาหรือตัวเหลืองได้ มีปัสสาวะมีสีดำ สีโค้ก-เป๊ปซี่ จากการที่มีเม็ดเลือดแดงแตก ปริมาณปัสสาวะอาจน้อยจนอาจนำไปสู่ภาวะไตวายเฉียบพลัน
จากการวิจัยพบว่าไม่ใช่ทุกคนที่มีพร่องเอนไซม์จีซิกซ์พีดี จะเกิดภาวะ เม็ดเลือดแดงแตกเมื่อกินถั่วปากอ้า แต่หมอจะแนะนำว่าควรหลีกเลี่ยง เพราะไม่มีใครสามารถคาดเดาได้ว่าจะเกิดกับเราไหม เราก็ใช้ชีวิตได้ตามปกตินี่แหละครับไม่ต้องกังวลอะไร เราคือเพื่อนกัลล์ 555

รายชื่อยาที่ผู้ป่วยภาวะพร่องเอนไซม์ G6PD ควรหลีกเลี่ยงได้แก่
1. กลุ่มยาแก้ปวด ลดไข้ ได้แก่ Aspirin, Aminopyrine, Dipyrone(Metamizole), Phenacetin
2. กลุ่มยารักษาโรคมาลาเรีย ได้แก่ Chloroquine, Quinine, Primaquine, Hydroxychloroquine
3. กลุ่มยาปฏิชีวนะ ได้แก่ ยากลุ่ม Quinolone, Nitroturan, Chloramphenical
4. กลุ่มยาเคมีบำบัด เช่น Doxorubicin
4. ยากลุ่มซัลฟา เช่น Dapsone, Co-trimoxazole
5. ยาโรคหัวใจ และหลอดเลือด ได้แก่ Procainamide, Quinidine และDopamine
6. อื่น ๆ ได้แก่ Vitamin C, Vitamin K (Menadione, Phytomenadione), Methylene blue, Toluidine blue, สารหนู และ Naphthalene
