    清醒不喘的缺氧新冠肺炎病人(silent hypoxia),為何突然猝死? (高明見教授)

    最近一連發生多件染病者,驚傳猝死家中或到医院前死亡然後確診,都歸為隐形缺氧所致,有多位專家解釋其致命機制,尤以12日長庚的高國晉醫師說明的最詳細,但都没說明为什麼清醒,應對如常的人,會突然猝死,我现在補充說明,讓大家更瞭解。

    ㈠病毒入侵肺部,肺泡有(ACE-2)引起發炎,導致換氣功能变差,造成血氧低下。

    ㈡大腦特有自主調控血流(Autoregulation)的功能,當血氧下降或二氧化碳升高,腦血管就會擴張(vasodilatation),增加血流,提供足够的氧氣和葡萄糖,維持腦的功能,所以雖然周邊組織(指尖血氧機)顯示己缺氧,病人仍可維持清醒,應對如常。

    ㈢肺炎會引起肺血流右向左的分流(R to L shunt),導致低血氧,但血中CO2卻不明顯升高。

    (四)呼吸功能主要受周邊化學感受器(頸動脈体Carotid bodies)及中樞化學感受器(延髓呼吸中心)的影響,以上兩感受器可能受病毒感染而降低其敏感性,雖有低血氧的刺激,病人不會自觉或引起氣喘。所以病人在相當缺氧下,不會氣喘,但腦血流仍维持正常,以致清醒行動如常.

    但周邊組織己是缺氧,尤其心臟特別需要充分供血,當周邊血氧一旦再下降,超過臨界點,對有心血管慢性病的高龄者或有交感神經亢奮的青壮年,可能因心臟急性供血不足,就可能導致心律不整,尤其心室顫動,而在沒有任何掙扎之下瞬間就猝死。

    (1)有30% COVID-19 silent hypoxemia 的病人自己不會覺得喘,外觀也看不出來 (除非測SpO2)。

    (2)Silent hypoxemia 病人見看醫生時常主訴extra-pulmonary symptoms, 例如 疲勞, 沒有胃口,瀉肚子。(病人外表常看起來一副輕鬆,如果在醫院外你甚至可能看不出他是一個病人)

    Salt, especially iodized (processed with iodine) is terrible for our skin. It causes the tissues to swell, making us look puffy and even a little unhealthy. It can aggravate acne, so it's important to reduce salt intake. Most of the salt we consume doesn't come from a shaker, so check the labels before buying packaged foods and snacks.
