2021-08-13 21:06:48This summer, we've been to Amagi Kogen 🗻 It is an Onsen (hot spring) resort located at an altitude of 1000m. The view of “Mt. Fuji, Suruga Bay / Sagami Bay” from here is spectacular! There is no obstruction around 360 degrees and it is a great place to see the “starry sky". Many activities for the whole family, from children to the elderly!

2019-11-03 23:15:31河内Hanoi是越南的首度，也是所有办公楼聚集的地方。根据我自己的经验，河内的居民比较的有礼貌，而且他们的英文程度也比较好，可以用简单（甚至是好）的英语和大家沟通哦，所以大家来到河内使用英语交谈不会是什么大问题啦！#Vietnamese #Hanoi #Backpacker #自由行
接下来就让我们一起来看看我在 2019年9月份前往河内Hanoi的5天4夜旅程吧，
除了有2天1夜的时间是在 下龙湾 Halong Bay 度过之外呢，其他的时间都是在河内的城市地区游玩哦。以下就是我建议几个在河内市区内的旅游景点：
Hanoi in the Capital of Vietnam, and most of the government office are located here. Building in Hanoi are very pack and transportation around the city is very busy. Most of their people ride motorcycle to avoid the traffic jam during office hours. Based on my previous travel to Ho Chi Minh City, the people in Hanoi is much more friendly. Most of the youngster can speak fluent English and we have no obstruction in language at all.
In this vlog, I have also included some of the scenery in Halong Bay. We have spend 2D1N in Halong Bay and the experience is fantastic. You must not missed out when you travel to Hanoi next time! Below are some of the places that I have visited during my travel to Hanoi. I hope it helps you in your next staycation in Hanoi! Have fun and Enjoy!

這有趣了… 韓國的非核家園政策看來有機會因為新政治態勢出現轉圜。總統候選人前檢查總長尹錫悅在與核工教授以及學生們路續會面與交流後，提出了政見，正式表明當選後將會廢除文在寅的廢核家園政策~~~ 而尹的民調是目前所有候選人中最高的，明年成為新任總統不無可能。
尹的參選理由與現任政府干預能源專業也有幾分相關，如週末的發文中提到，政府閣員干預電力公司營運使核電廠提前關閉的醜聞已被起訴，而當初挺住壓力決定要查這個案子的就是時任總長尹錫悅。

今日寫完個post發現好多人覺得只要病人或家屬想要，醫護就要尊重同服從。
但其實條線並非想像中咁簡單，舉幾個常見嘅兩難
1. 最常見的不作心肺復蘇CPR
其實法律上這個是一個醫療決定，醫生可以直接決定要不要CPR。但在晚期癌症或無可逆轉的長期疾病的病人上，進行CPR很多時候只會造成更大創傷而對生活質素或預後並無改善。
醫生跟病人和家屬一起商討是希望大家一起下shared decision，令大家明白為何醫護人員傾向不作CPR，好讓大家不會覺得有遺憾。
晚期病人好多CPR同插完喉，一試圖extubate就轉差，只能依靠呼吸機生存係唔係有品質嘅生活？
插喉插得耐又會易感染肺炎，敗血症時落唔落抗生素、落唔落強心針？落完但又甩唔到條喉時，又搞一輪之後再感染又敗血症，多重器官衰竭，係咪病人或家屬話要救，又再落抗生素同強心針、升壓劑等等？
無生活質素，需要依賴機器生存嘅時候，係咪對病人好呢？如果純粹因為病人或家屬話要，就隨便服從、我唔認為係負責任嘅做法
2. 輸液IV fluid與否
好多人覺得病人想要就應該俾，但其實IV fluid都係一個醫療決定，醫生同樣可以直接決定俾唔俾。的確醫生寫兩隻字唔難，輸液亦唔係咩高風險嘅治療。但其實對晚期紓緩治療嘅病人來講，要唔要輸液真係一個兩難。
好多病人無胃口進食困難，或因癌症壓迫食道或腸胃道而無法進食。的確你可以一直輸液buy time，但輸液要有IV access，唔係個個病人都有條PICC line或PORT可以長期輸液，打豆嘅話粒豆每幾日又要換。
老人家或者晚期癌症病人，特別有bone narrow infiltration嘅話，血小板唔高，次次打豆都會打到成身瘀晒。每幾日又要換豆，好多晚期病人本身都唔易打，又要打幾次先上，成身d靜脈都用哂去打豆，成身都瘀哂，對病人同家屬來講又係唔係容易消化同承受嘅事？
你輸液我當你慢輸80ml/hr，你每日要換兩包水，粒豆有時downstream obstruction部機又係咁beep beep叫，護士又要幫你調整下粒豆，每次都會整醒你，一日俾人整醒十幾廿次嘅生活又係唔係有質素呢？
輸得液嘅時候，好多醫院都要求有regular vital signs obs，我當你每日6次度血壓量心跳，又要整醒你搞你一輪。
但你進食唔到嘅病人就會覺得個人好乾，就算每日護士幫手做mouth care，都未必足夠，咁嘅時候要唔要輸液呢？輸完又wean唔Wean到？wean唔到係唔係要輸到2046到病人臨終嘅一日？
3. 病人同家屬決定就好，醫護無權幫人作決定
其實好多醫療決定，如果無醫護人員諮詢建議嘅話，只係想個責任同壓力拋俾病人同家屬。純粹係但求可以輕鬆覺得既然佢地都決定咗，就咁啦，尊重病人如家屬就好。
我認為重大醫療決定唔應該係單方面醫生或病人或家屬去做。而係一個shared decision，醫護人員盡力進行諮詢，係尊重病人同家屬意願同時，亦要以病人實際生活質素去考慮。
但呢個講就易，實際操作時其實好難
