硃砂是毒性中藥之一

事實上，台灣行政院衛福部(當時的衛生署)自94年5月1日起禁止中藥用硃砂製造、調劑、輸入、輸出、販賣或陳列。

毒性中藥

有些毒性中藥的藥性十分強烈， 因為含有毒成分，所以若「藥量稍不當或誤服」，即可引致中毒，甚至死亡。

「毒性中藥中毒」

當中毒時，我們身體可能會出現單一器官或多重器官系統受影響，可能的急性症狀如下 (從頭到腳都有，渾身不舒服) :

1. 神經系統症狀：例如有dizziness、headache、seizure、limb numbness、blurred vision、hearing impairment，coma or paralysis。

2. 呼吸系統症狀：例如有dyspnea、hoarseness or liporal cyanosis 。

3. 消化系統症狀：例如有nausea、vomiting、burn-like abdominal pain、jaundice、diarrhea，hematemesis or bloody stool。

4. 泌尿系統症狀：例如有hematuria、edema、dysuria。

5. 循環系統症狀：例如有arrhythmia、chest pain or tightness。

｜病因｜


哮吼 (croup) ，全名是「喉頭氣管支氣管炎 (laryngotracheal bronchitis)」， 是常見的幼童上呼吸道感染疾病，副流感第一和第三型病毒 (parainfluenza virus types 1 and 3) 是主因，其餘的感染源是 A 型流感病毒 (influenza A)、呼吸道融合病毒 (RSV：respiratory syncytial virus) 、腺病毒 (adenovirus)。


｜症狀和診斷｜


好發於 2 歲以下的幼童，病毒感染造成的發炎反應會引發聲門下呼吸道水腫甚至阻塞，症狀從輕微到嚴重都有，較為嚴重的病人會有明顯的吸氣喘鳴聲 (inspiratory stridor)，聲音沙啞 (hoarseness) 和狗吠聲咳嗽 (barking cough) 。在最嚴重的病人身上會發現呼吸時有胸骨上凹陷 (suprasternal retraction) 和肋骨間凹陷 (intercostal retraction)。此外，大部分哮吼病人在出現上述典型症狀前會出現低溫發燒、咳嗽、流鼻水等一般感冒症狀。


診斷主要依靠病史和醫師的理學檢查，頸部 X 光可能出現「尖塔狀 ( steeple sign)」，可以幫忙診斷，但不能是唯一的診斷依據。通常不會流口水 (drooling)，若出現，要和會厭炎 (epiglottitis) 做鑑別診斷。


｜治療方式｜


症狀輕微的哮吼病人，必須好好補充水分，並需要吸潮濕的空氣潤滑呼吸道和讓痰液不要太黏稠。


病人休息時喘鳴聲或呼吸時胸骨凹陷，是較為嚴重的哮吼病人，吸入「氣化腎上腺素」收縮黏膜血管，幫助呼吸道黏膜消腫緩解症狀，但治療後建議在醫院觀察 2 至 6 小時，觀察可能發生的「症狀反彈現象」。注射或是口服類固醇是安全有效的輔助治療，可以降低需要插管的機率。


嚴重的哮吼，通常需要住院觀察。萬一病人嚴重到需要插管急救，因為呼吸道水腫狹窄，氣管內管的大小建議選擇比正常大小「小半號」。反覆哮吼或插管後困難拔管，建議做「喉鏡和支氣管鏡」排除其他病因。
