#เล็งมาเล่า เพราะเราได้คีโมมาครบ 1 ปีแล้ว . วันนี้เมื่อปีที่แล้ว ผมเข้ารับการทำ #TACE ที่รพ.จุฬา​ฯ​ ซึ่งคำว่า เทส ย่อมาจาก Trans Arterial Chemo Embolization หรือการรักษามะเร็งตับ ที่ผู้ป่วยที่ไม่สามารถผ่าตัดทิ้งได้ โดยการรักษาด้วยการให้เคมีบำบัดเฉพาะทางที่ผ่านทางหลอดเลือดแดงที่ไปเลี้ยงก้อนมะเร็งตับโดยตรง แล้วอุดกั้นหลอดเลือดนั้นเพื่อไม่ให้เลือดไปเลี้ยงมะเร็ง และไม่ให้เคมีออกมาส่วนอื่นๆของอวัยวะ . หมอไม่สามารถ​ตัดก้อนมะเร็ง 4*4*4 ซม.กว่าๆทิ้งแบบเคสอื่นๆ เพราะมีอีกหลายร้อยเม็ดเล็กๆเต็มทั่วตับ ทางอจ.ธีระ พิรัชวิสุทธิ์ และ อจ.ณัชชา ปิ่นเจริญ​ และทีมแพทย์ จึงเลือกวิธีนี้ . ห้ามหลับ ขณะรับการรักษาครับ หมอฝนแทงเข็มนำร่องเข้าเส้นเลือดใหญ่ระหว่างขา(ขวา)​ ใช้ฝีมือและประสบการณ์​ของแพทย์ล้วนๆในการเดินตามเส้นเลือดไปถึงตับ มียาชา จึงไม่เจ็บ แค่จุกๆครับ ทำอยู่เกือบ 3 ชม. เราได้เห็นทุกขั้นตอนแบบเรียลทามบนจอ ประหนึ่ง​เราคือโทนี่ สตาร์ก ที่กำลังถูกหมอเอาสะเก็ดระเบิด​ (Shrapnel)​ ออกจากร่าง หรือ วูฟเวอรีน ที่กำลังถูกฉีด อดาแมนเทียม เข้าร่างกาย ทำให้ผมรู้สึก​ไร้เทียมทานทันที 55555 . ออกจากห้องผ่ามา ต้องนอนพักฟื้นอย่างน้อย 4 ชม. ถึงจะลุกได้ อยากเข้าห้องน้ำไปนั่งถ่ายหนักเบา อยากสั่งข้าวมากิน แต่ต้องรอเวลา ตั้งนาฬิกาปลุกกันเลยทีเดียว ท่อนล่างชาพอควรเลยครับ เพราะต้องนอนรอโดยห้ามขยับขาจนกว่าจะครบกำหนด . และแล้วมะเร็งตับก้อนใหญ่เท่ากำปั้น​เด็กก็ฝ่อลง แต่ไม่หมดครับ รอบถัดไป จึงต้องทำ อะ-บเล-ชั่น (Ablation)​ หรือการจี้ร้อนด้วยคลื่นวิทยุ (Radio​ Frequency) แล้วจะมาเล่าให้ฟังว่ารู้สึกอย่างไรครับ . ค่าใช้จ่ายเท่าไร ขึ้นกับความยากง่าย สถานที่ เครื่องมือครับ ผมขอไม่ไปแตะ เพราะมักมีการเปลี่ยนแปลง​อยู่เสมอจากหลายปัจจัย​ . #มะเร็ง #มะเร็งตับ
