抗凝血藥物Heparin

肝素(Heparin) (unfractionated heparin, UFH)：

傳統使用的抗凝劑，Heparin 是酸性帶負電粘多醣類，分子量大。
作用機轉為 Heparin 加強抗凝血酶去活性，可與凝血因子Ⅱa、Ⅹa、XⅡa結合，使其失去活性，特別是對凝血酶子。

臨床用途： Heparin 是最主要的抗血栓藥，治療靜脈栓塞、預防手術後血管栓塞。在體內體外均有抗凝血作用，作為體外抗凝血劑。需監測病人的aPTT(活化部分凝血活素時間, activated Partial Thromboplastin Time)

使用劑量：60 U/kg IV bolus, (最大劑量：4000 U), 12-14 U/kg/hr

深部靜脈栓塞(DVT)：
預防性：
5000 units SC q8-12hr, OR
7500 units SC q12hr

急性冠心症(ACS)
1、最初推注劑量：每公斤60單位。(最大劑量4000單位)
2、每小時每公斤滴注12單位，共48小時。
3、維持APTT於控制值之1.5~2倍。
4、於6, 12, 18, 24小時，檢查其APTT值。
5、體重小於70公斤者，滴注劑量為每小時1000單位。
6、合併使用GP IIb/IIIa 抑制劑，劑量：50- 70 U/kg IV bolus
7、未合併使用GP IIb/IIIa 抑制劑，劑量： 70-100 U/kg bolus

Heparin副作用：

1、最大的副作用是出血。因此須監測病人的aPTT，若監測值大於正常範圍的 1.5~2.0倍須減量或停用。
2、使用36小時後，Heparin 與血小板形成複合體，使血小板濃度減少，長期使用(6個月)會使骨鈣流失出現骨骼疏鬆現象。
3、出血，過敏反應(冷顫、蕁麻疹、發燒、鼻炎、氣喘樣的反應)，急性可逆性血小板過少症，禿頭，骨稀鬆病、腎功能受損，SC和IM注射部位局部刺激。

Heparin注射過量時的解毒劑：

Protamine 1 mg IV 可中和Heparin 100 U, 最大劑量50mg。靜脈緩慢注射：<20 mg/min,且<50mg in 10 min

如果Heparin是用 IV dripping，就以開始注射時間的長度來換算Protamine的劑量：

Heparin已經滴注30-60分鐘，則使用Protamine 0.5-0.75 mg 可中和Heparin 100 U。

Heparin已經滴注超過2小時，則使用Protamine 0.25-0.375 mg。

Heparin交互作用

1.協同作用：Acetyl Salicylic acid, Dipyridamole, Phenylbutazone,
Indomethacin、Sulfinpyrazone、Probenecid、Dextran、Ethacrynic acid、Dicumarol、Cytostatic drugs。

2.拮抗作用：Antihistamines、Digitalis、Tetracycline、Nicotine abuse、Ascorbic acid。

3. 若同時併用丹參、當歸、川紅花、藏紅花併用，可能會增強抗凝血作用。

4. 若同時併用吉林參、吉林參鬚、高麗參、西洋參併用，可能會降低抗凝血作用。

Heparin禁忌

1.禁用於下列情況之患者：血友病、急性胃腸道出血性潰瘍、代償失調引起的嚴重高血壓症、中樞神經系統外科手術上、疑似腦出血症、漸近性心內膜炎、甲狀腺手術、疑似惡性腫瘤而有出血危險者、
2.對Heparin Sodium過敏者。

Heparin注意事項

Heparin要換成Warfarin時，需要5天同時使用，檢測INR 維持於2.0-3.0，連續2天，然後停掉Heparin 或LMWH改用Warfarin。

在所有臨床檢驗的離子裡面，鎂是最少被醫生注意到的，甚至血鎂這個檢驗項目經常會被健保局核刪。

血鎂的 60% 是游離的，10% 與陰離子結合，30% 與蛋白質結合。只有游離的鎂具有生物活性，但是大部分的血鎂濃度只有測定鎂總量（正常值 1.7-2.7 mg/dL）。而且大部分的鎂在細胞內，也能在細胞內外移動 ，因此血鎂不一定能代表身體的鎂含量。

鎂是人體中第四多的陽離子（最多的是鈣、鉀、鈉），也是細胞內第二多的陽離子（僅次於鉀）。50-65% 在骨骼，20% 在肌肉內，只有 1% 的鎂在細胞外液。

低血鎂的危險因素是胃腸疾病（慢性腹瀉、嘔吐、鼻胃管、腸造口或瘻管、急性胰臟炎）、酗酒、營養不良、furosemide、thiazide、藥物（PPI《抑制小腸吸收鎂》、胰島素、cisplatin、aminoglycoside、amphotericin、cyclosporine、digitalis、EGFR 抑制劑 、MMF）、泌尿道阻塞解除後利尿、內分泌疾病（糖尿病、甲狀腺高能症、副甲狀腺高能症、原發性高醛固酮症）、SIADH、高血鈣、低血磷、代謝性酸中毒、腎小管疾病（吉特曼症候群、巴特氏症候群）、急性腎小管壞死的恢復期、餓骨症候群、急重症、大量輸血等。

當血鎂濃度正常時，也可能有鎂缺乏。例如：當病人有補充鉀無效的低血鉀或是補充鈣無效的低血鈣時，此時可以用補充鎂來嘗試治療。

低血鎂的症狀是肌肉無力、痙攣、意識障礙等。缺鎂的併發症是低血鈣、低血鉀、心律不整、高血壓、冠心症、骨質疏鬆、泌尿道結石、糖尿病等。

雖然亨利氏環吸收最多的鎂，但是奇怪的是吉特曼症候群（缺乏 TRPM6，類似 thiazides 的效果）比巴特氏症候群（缺乏 NKCC2，類似 furosemide 的效果）更容易低血鎂，這是因為巴特氏症候群時遠端腎小管會代償性增加鎂的主動吸收。

Parfums Quartana Review, in this video I’m going to give you an overview of the house, going through the scent and giving you my quick thoughts on them. First I’ll start off with the ones that I fell in love with and will definitely be getting, next are the ones that I enjoyed but won’t necessary buy, and lastly are the ones that I didn’t really like.

FRAGRANCE MENTIONED

- Mandrake- Mandrake is a very amazingly juicy and tasty fragrance, this was love at first sniff for me. Great for hangouts and any outdoor events because of how piercing and strong it is. You’ll definitely get notice with this one.

- Wolfsbane- Wolfsbane is a dirty gourmand, it has an earthy green vibe to it that I don’t particularly enjoy but I do, however, really enjoy the gourmand aspect of the fragrance, which I could only get when I smell the silage. Up close this scent is not so appealing.

- Hemlock- Hemlock is another favorite of mine. It’s such a beautiful powdery white floral that gives off such a calming effect. I would really love to own this but the reason I won’t be buying this is because I simply cannot figure out a good time to wear it other than before bed, which I don’t really feel like it would be worth the $145 price tag if that’s all I’m using it for.

- Digitalis- This is a very simple clean and green scent. Derives mostly of cucumber and basil, this scent gets brighter and brighter as it goes, which is very strange because usually scent gets duller as time passes. Since I’m not a fan of green scent this one doesn’t appeal to me too well, still not a bad scent, it’s just not for me.

- Blood Flower- This is very similar to Bull’s Blood by Imaginary Authors. It has a very rosy vibe with a touch of blood like accord. It smells of irony resembling what real blood smells like but not in a gross way at all. The licorice in this adds a nice candy vibe that I do enjoy, but personally, this is probably the hardest one to wear and for that reason alone, I can’t see myself buying it.

Overall Parfums Quartana is fairly unique on how they approach their scents and the product that came out are prove of their creativity. The only downside to them is really just how daring the scent are. A lot of them are simply hard to wear.
