【科普文分享】真假難分 － 2019 冠狀病毒病相關假性血小板減少症／史丹福狂想曲

／／最先在中國武漢出現的 2019 冠狀病毒病 (coronavirus disease 2019) 肆虐全球，造成大量感染與死亡。

其實此疾病亦以不同的方法影響血液。近來的血液學期刊都充滿著與 2019 冠狀病毒病相關的文章，例如 eJHaem 血液學期刊最近就刊登了一個 2019 冠狀病毒病引起的假性血小板減少症 (pseudothrombocytopenia) 。

病人是一位 54 歲的女士，她在患病前血小板數量正常。在症狀出現後的第 10 天，她的呼吸系統症狀加劇。如此同時，她的血小板數量由原本的 236 x 10^9/L 急跌至 6 x 10^9/L 。

化驗師及醫生在這情況下都必定會以顯微鏡檢查病人的周邊血液抹片，以找出血小板數量快速減少的原因。文章的作者發現病人的血小板其實並沒有顯著減少，只是粘在一起，形成血小板凝塊 (platelet clumps) 。自動血液分析儀並不夠聰明，不能夠分辨黏在一起的血小板，它們不會被數成血小板，所以這只是一個假的血小板減少。血小板凝塊的成因是收集血液的血瓶中有EDTA抗凝血劑去防止血液在運送或儲存期間凝固，如病人體內有抗 EDTA 的抗體，就可引起血小板凝塊。解決方法是試用另一款血瓶抗凝血劑，例如檸檬酸。案例中的病人轉用檸檬酸血瓶後發現血小板數量有 129 x 10^9/L。

病人的 2019 冠狀病毒病後來康復了，血小板數量亦回升，在 8 星期後， EDTA 血瓶量度的血小板回升至 99 x 10^9/L。這顯示病人的血小板凝塊與假性血小板減少症是一個與 2019 冠狀病毒病相關的暫時性現象。

For this episode, we headed back to Neo’s Kitchen to attempt a LEVEL 99 SPICY MALA WONTON CHALLENGE! EB Food is organising a timed wonton challenge with the launch of their new product, EB Shrimp Wonton with Mala Sauce. Eating the greatest number of EB wontons in 5 minutes will net you a cash prize of $2000, with further cash prizes for 2nd and 3rd place. Full details listed below:

The EB Shrimp Wonton with Mala Sauce came neatly packed in a pleasantly-enough designed box that really undersold the potency of the Mala sauce. They were simple to cook - boil the 12 wontons for 5 minutes, warm up the Mala sauce in hot water and mix them all together. The Mala sauce was a menacing shade of red glistening with Mala oil radiating that signature Mala aroma. The wontons themselves respectably plumped up considerably during cooking. Each had a whole shrimp in them. When mixed together, each wonton ended up adopting an even shade of red with small clumps of Mala sauce noticeable throughout.

I attempted this challenge with Mervin (there is a buddy requirement). From the number of wontons we prepared versus the number consumed, it was clear that we underestimated this challenge. We thought it would be a speed challenge, but it ended up being a spice challenge due to our low spice tolerance and the unexpected heat of the Mala sauce. Out of the 100+ wontons we cooked, we only ate 48 between us within the 5 minutes limit.

Note that our attempt does not count for the official challenge, so please do find a buddy (preferably someone who loves Mala) and win yourself $2000! The wontons are also Halal, so Muslim viewers can join as well!

平たく焼いたタルトにクリームとフルーツをのせた簡単タルト作りました。フルーツとの相性がめちゃいいクリームは、さっぱり味に仕上げました。余ったタルト生地は、焼き上がりをそのまま食べても美味しいので、使い切って仕上げの飾りに使うなど自由にアレンジしてみてください。
*レシピ*
タルト生地作ります
１．無塩バター 50gを柔らかくする。
２．グラニュー糖（上白糖でも）30g、塩 ひとつまみを入れ混ぜる。
３．卵黄 1個分を入れ混ぜる。
４．薄力粉 100g、アーモンドパウダー 20gをふるい入れさっくりと混ぜる。
５．ひとまとまりにしてラップに包み、1時間以上冷蔵庫で休ませる。
６．５ミリの厚さに伸ばし、18センチの円に抜く。
７．８等分にし、フォークで穴をあける（ピケする）。
８．180度に予熱したオーブンで20分焼く。
９．冷ます。
余った生地はタルトの上にのせる飾り用の顔にしてみました。焼成は、タルトと同じで180度で20分。ピケで膨らみを抑えられないので、天板などを上において焼いてください。

クリームチーズクリームを作ります。
１０．クリームチーズ 50gを柔らかくする。
１１．砂糖 20g、お好みの洋酒 10gを順に入れ、混ぜる。
１２．生クリーム 120gを少しずつ加えながら混ぜる。
１３．少しかたさが出るまで泡立てる。
１４．９に１３をのせる。
１５．フルーツなどで飾る。
１６．よく冷やしてからいただく。

We baked a simple, flat tart topped with cream and fruits. The cream really has the right chemistry with the fruits, giving it a refreshing flavor.The leftover dough tastes good baked as is, so please use it as you like, such as by using it to create some finishing touches.

*Recipe*

Making the tart dough.

1. Soften 50 grams of unsalted butter.
2. Mix a pinch of salt with 30 grams of granulated sugar (you may also use powdered sugar).
3. Mix in 1 egg yolk.
4. Strain 100 grams of wheat flour of low viscosity and twenty grams of almond powder through a sieve, and then lightly stir the mixture.
5. Wrap the mixture in a plastic wrap and refrigerate it for more than 1 hour.
6. Pull out clumps with a circumference of 18 centimeters, and spread them evenly with a thickness of 5 millimeters.
7. Around 8 minutes later, make holes in them using a fork (pique it).
8. Preheat the oven to 180 degrees, and bake for 20 minutes.
9. Let it cool down.

We tried using the leftover dough to make decorative faces for the top of the tart. You can bake it for as long as the tart, which is 20 minutes at 180 degrees. With a pastry pricker, the rising of the dough is not held in check, so please place and bake the dough on a baking tray or something similar.

Making the cream cheese frosting.

10. Soften 50 grams of cream cheese.
11. Pour 20 grams of sugar, followed by 10 grams of your Western liquor of choice, onto the cream cheese, and then stir.
12. While stirring, add 120 grams of fresh cream, a little at a time.
13. Whip until the mixture slightly hardens.
14. Place 13 on top of 9.
15. Decorate with fruits.
16. Consume the tart after letting it cool down sufficiently.
