Perampanel 起始劑量該怎麼給？

Perampanel (癲控達，Fycompa) 為 2012 年上市的抗癲癇藥 (AED)：
▋藥理機轉：選擇性、非競爭性之 ionotropic AMPA glutamate receptor 抑制劑。
▋適應症：適用於 12 歲以上之成人或青少年
☞ 局部癲癇發作合併或未合併續發型全身發作之輔助治療 (focal onset seizure; adjunct)
☞ 原發型全身性強直陣攣癲癇發作併有原發性全身發作之輔助治療 (primary generalized tonic-clonic seizure; adjunct)

如果有一位同時併用 carbamazepine, levetiracetam, lamotrigine 與 lorazepam inj，但癲癇還是持續發作的皮卡丘，此時欲加入 perampanel 作為輔助治療，則其建議起始劑量該怎麼處方呢？
▋Perampanel 與 lamotrigine 很像，都是需要看當下併用的 AED 是否有影響到自己的代謝路徑，來決定自己的起始劑量及 titration interval ，完全就是個看別人臉色的孩子~
▋Perampanel 主要的代謝路徑為 CYP3A4 (major), 1A2 (minor), 2B6 (minor)。
▋當 perampanel 併用 carbamazepine, oxcarbazepine, phenytoin (and fosphenytoin) 這 3 - 4 個主要的 enzyme-inducing AED 時，起始劑量需要從 2 mg hs 提高到 4 mg hs，。
▋因此，這隻併用 carbamazepine 的皮卡丘，perampanel 的起始劑量應從 4 mg hs 開始投予（Drug interaction: [Severity: major][Documentation: fair]），並觀察神經學症狀予以每週調升 2 mg，直到到達極量 12 mg/day 或無法耐受。

只能說，大難不死必有後福啊...(被質子束打到的機率?)
Bugorski was working as a researcher at the Institute for High Energy Physics in Protvino in 1978. At the time, he was working with the the largest Soviet particle accelerator at that time, the U-70 synchrotron. He was checking a malfunctioning piece of equipment when safety protocols failed, and he managed to accidentally stick his head in the path of the proton beam. According to his own report, he felt no pain but saw a flash brighter than a thousand suns.
After the accident, the left half of his face swelled up and the skin began to peel. Having received a huge dose of radiation, he was not expected to survive. However, he made a full recovery and even completed his PhD. He is still alive today, and the only long term side effects seem to be deafness in his left ear, increased fatigue, occasional complex partial seizures and rare tonic-clonic seizures.
