整形外科讓你意想不到手術: 第二彈!!!!!!!!

大家好，今天朱醫師要來介紹一個你意想不到的手術，而這樣的手術在國內外都是由整形外科和骨科醫師共同合作完成的唷!!
是什麼奇幻花俏的手術呢?? ^^

大概一個月前，一個年輕男性因為車禍，從大腿到膝蓋以下的部位整個創傷性截斷，來急診時，就是一個從膝蓋以下斷開的腿，膝蓋關節也已經粉碎性骨折。即使直接接回去，膝關節也不可能有彎曲的功能。然而，不能彎曲的膝蓋，是無法讓人行走的，換句話說整個殘肢是保不住了!
這時，說時遲那時快，我們科經驗豐富的主任，馬上想到一個可以重建患者傷口，並且往後可以順利穿戴義肢的方法!! 那就是
"Rotationplasty" 中文沒有特別翻譯 XD

(可能可以翻成" 旋轉整形??" 之類的吧XD)

首先骨科醫師將壓毀的膝關節去掉，保留沒有被壓爛的小腿到腳踝，轉個180度，對，你沒看錯!! 轉個180度，接到大腿上，再由整形外科醫師把血管和神經進行接合，手術就完成了!!
聽起來很奇怪，為什麼要這麼做??
原來，是要用踝關節的活動度，來取代已經粉碎、不能使用的膝關節，讓病人有機會穿上義肢，重新站起來，走向未來的人生!
即使是最厲害的整形外傷科和骨科團隊聯合進行，也花了一整天的時間，才成功地結束這罕見困難的手術。
術前，我們主任也和病人以及家屬認真地解釋這個手術的方式步驟及用意。手術也十分順利!
然而，手術結束後，加護病房裡....
「醫師，你們為什麼把我兒子的腳接反了!」病人母親既生氣又焦慮地對著醫療團隊質問!
「我們要使用你兒子的踝關節來取代膝關節.....」主任又解釋了一次!
「你說什麼我都聽不懂，我叫我先生來!」病人母親實在無法接受自己的孩子的腳被接成這樣!
病人的父親來了! 終於可以溝通~
「這是介紹的影片，關於這個手術的用意....」團隊苦口婆心地再解釋一次~
老實說，這些方法在手術前，都已經解釋過了，但是當下情況危急，病人和家屬根本就聽不進去到底要進行什麼手術，也沒有心情去理解，只一味地催促醫師趕快安排，於是術後就形成這樣尷尬的局面~~
病人本人躺在加護病房時，一臉鎮定，好像不置可否，彷彿已經接受了這個結果。
好不容易出了加護病房，過了五天，病人突然某天早上抓著我們專科護理師的手，悲傷地問:
「我的腳，是不是醫師搞錯逗反了? 」

(@@這位先生，原來你從頭到尾的冷靜都不是因為出於了解與接受，原來是10幾天來你都沒有正眼看過你的腳@@)
於是同一支影片，又拿出來，讓病患看了幾遍!!

現在，病患傷口漸漸癒合好，也快要可以穿戴義肢了! 雖然理解和接受需要時間，但是整形外傷科的醫師和骨科醫師，在有限及逼不得已的身體條件下，做了最好的選擇。希望這位有為青年，可以慢慢藉由復健重新站起來，邁向自己未來光明的人生!

"It's Okay" is an original song written by Lisa Hui in May 2016. Lisa had given up her beloved musical world for her love and new born son. The change in circumstance left Lisa very depressed at the time. This song was written for the "9th Chinese Song Writing Competition", sponsored by the Canadian Chinese radio station.

Lisa took the opportunity to write and record "It's Okay" while rejoicing in her return to music.

"It's Okay" describes a girl who is reborn to create music... capturing Lisa's return to writing original songs. The story of a woman who's experiences through a false world of love left her without her creativity. Leaving her alone in a place where no matter how hard she tries the reality of lost love leaves her on the edge. The reality of life has hit her hard.

Today, this girl wakes up from the lies, grows and transforms the pain into strength. She learns to let go and return to her beloved musical world again. Standing on the stage and shining again is where she wants to be. She has realized that as long as she walks in the world of music the pain and hurt are no longer important... "It's Okay"

Lisa hopes that her audience and listeners feel the truth and the life in her music. No matter how much damage or trauma we have experienced as long as there is music "It's Okay"

〈無垢鳥〉的歌詞係由《維摩詰所說經·方便品》以及客語童謠〈阿啾箭〉所發想而成。
The lyrics are inspired by “The Vimalakirti Sutra” and the Hakka nursery rhyme “Black Drongo”.

阿啾箭為大卷尾的客語稱法，是地域性很強的鳥類，昔常見於農村牛背上；維摩的梵語是清淨、無垢的意思，結合以上二者，故曲名題為〈無垢鳥〉。
A black drongo is a highly territorial bird often seen resting on the back of a cow in the countryside. “Vima” in Sanskrit language means clean and pure, which is why the song is named “White Drongo”.

〈阿啾箭〉這首童謠使用在電影《光的孩子》的劇情關鍵處，藉由不同角色的無伴奏哼唱，前後串連出生命、情感的一脈相承。〈阿啾箭〉的童謠歌詞迴環反覆、直白簡要，指出無父無母的處境、追尋的失落，剛好扣合電影的角色際遇，也與《維摩詰所說經·方便品》的核心精神相呼應。
The Hakka nursery rhyme “Black Drongo” is used in an important part of the film “The Child of Light”. Hummed by different characters without the accompaniment, it expresses how our lives and feelings are interrelated. With the rotatory and straight forward lyrics, we know how it feels when you’ve lost your parents and try to find something but in vain, which is what the protagonist of the film has been through and in accordance with the core of “The Vimalakirti Sutra”.

〈無垢鳥〉歌詞以聚沫、泡、燄、幻、夢、影、浮雲、電等喻，示明無常的諦理；而〈阿啾箭〉童謠則提供了一個生活感的基底；兩者一虛一實，精神層面和現實景況交匯共振，成為了普世無盡的扣問。
A cluster of foam, a bubble, a flame, an illusion, a dream, a shadow, a cloud and lightning in the lyrics show us how transitory life is while the nursery rhyme “Black Drongo” itself describes an ordinary daily routine. The former is unreal while the later is real. When the spirituality meets the reality, endless questions arise ubiquitously.

〈無垢鳥〉是電影《光的孩子》的片尾曲。原初的構想是希望片尾曲能有不斷往前、往前，最後隨風消散的感覺，意圖在影片結束時，以對自然的感知、經文的無常空相，延擴至更超然的識界，並帶給人飛昇、療癒的力量。
“White Drongo” is the ending song of the film “The Child of Light”. The initial conception about ending song is to deliver the feelings of going forward and fading away eventually. We expect that the ending shows the sense to nature and the impermanence of sutra expending to the unlimited ambit, and also provides people the power of inspiration and mind-curing.
