輝瑞/BNT疫苗未稀釋的事件
其實，我們要 #肯定 恩主公醫院的 #誠實告知
並願意為病人安全負責的態度
因為這是 #清點施打數量才發現異常

所以是經由告知才讓事情浮出檯面

各位清楚BNT疫苗怎麼稀釋與抽藥嗎?

BNT疫苗為原裝晶凍懸浮液小瓶，6-7人份濃縮在0.45cc
這樣的設計顯然是為了節省運送成本

所以使用者自然要花比較大的時間成本稀釋

1️⃣. 先解凍
讓小瓶在冰箱中解凍 3 小時，或讓小瓶在室溫下放置 30 分鐘
任何一種解凍方法，必須在稀釋前達到室溫，並且在2小時內稀釋

2️⃣. 對小瓶稀釋
對小瓶注入1.8cc的0.9%生理食鹽水
這樣就是稀釋體積到2.25cc，預計抽取出來的濃度降為1/5
並且反抽空氣

3️⃣. 標記稀釋時間
這個步驟其實蠻重要的
就是標記好稀釋時間點
因為稀釋後室溫下必須6個小時內完成施打，否則丟棄
也是告知該小瓶已經稀釋，可以安全抽藥

4️⃣. 抽藥
抽取給每個人劑量是0.3cc
6個人，也就是2.25cc會抽走1.8cc以上
但是護理師不是抽藥機器人，難免有誤差
導致第7人可能會劑量不足
所以這是BNT一瓶預留給6個人的原因
除非該護理師很客家人，才有可能真的抽到7*0.3=2.1cc

恩主公醫療人員是在清點時
發現施打數量才發現異常，因散裝部分疫苗瓶蓋掉落
可能就是3️⃣的部分當下沒check到

不過事情既然發生，我們可能會怎麼處理?

假設完全沒有稀釋，接種者可能打到了4-5倍的劑量
我們也不能說這相當於多打了一點點莫德納(為BNT濃度三倍)
因為同時還要考慮過量 #佐劑 的作用
這是會大大增加病人疫苗不適作用的關鍵原因

但是一般來說，藥物安全設計其實有"safe range"
都可以承受10倍正常劑量以內
就像安眠藥設計不要讓病人吞藥事件中致死
現行疫苗大多應該不會因為未經稀釋施打而造成死亡或重症

國外也是很多烏龍施打事件，也沒有後續有人死亡的追蹤報導
所以我想是不必要太焦慮太擔心

後續的處理方式
除了建議住院一週觀察副作用
另外就是驗抗體，看有沒有over stimulate(過度刺激)的作用
我想事情就可以結束了，社會不用過度的焦慮

我也呼籲要社會 #要肯定恩主公醫院誠實告知並交代事情來龍去脈
大量施打本來就是苦差事
這些包含清點、每次滾動改善流程都是複雜作業
醫護人員都是醫療回載下又被臨時徵召
壓力之大，就不要再拿放大鏡檢視一切
我們就讓異常醫療事件通報去走正常的程序去處理即可
