Iverson had 22 points, matched a season-high with nine assists, grabbed four steals and did not turn the ball over in a 90-79 win over the Portland Trail Blazers on Friday night.
"He had an amazing game, especially down the stretch when he got us some really good shots,'' Sixers coach Larry Brown said. "This is by far his best year, on both ends of the court.''
Iverson's performance came on the night when the 76ers honored
the 1982-83 championship team at halftime. He acknowledged that it
was a special night for him seeing the fans' reaction to his
predecessors.
"This was something that I will cherish for the rest of my
life,'' Iverson said. "This sends me another message of how
blessed I am. Hopefully one day I can come back and get the same
response.''
The Sixers improved to 13-2 since the All-Star break and snapped
Portland's three-game winning streak.
Keith Van Horn added 20 points and nine rebounds, Aaron McKie
had 13 points, Eric Snow scored 12 and Derrick Coleman grabbed 10
rebounds for the 76ers. Snow left the game briefly in the fourth
quarter with a lacerated right middle finger but returned after
receiving five stitches.
Damon Stoudamire had 16 points, Rasheed Wallace scored 15 and Dale Davis had 12 points and 12 rebounds for the Trail Blazers, who turned the ball over a season-high 25 times.
"That's the game right there, 25 turnovers,'' said Blazers coach Maurice Cheeks, who was part of the 1982-83 team. "When you turn it over 25 times, that makes it tough.''
Philadelphia scored 11 of the first 15 points of the fourth quarter and pulled ahead 75-65 when Van Horn slammed down an alley-oop pass from Iverson with 8:55 left.
The Trail Blazers pulled within six, but another dunk by Van
Horn off of a pass from Iverson capped a 6-2 run to extend the
Sixers' lead back to 10. Portland was unable to get closer than
eight in the final six minutes.
"It was a great team win, especially in that fourth quarter,''
said Brown. "When they came back, we played as a team. Allen made
some unbelievable plays, some great passes and we made some
stops.''
A 3-pointer by Wallace capped a 14-6 run to pull Portland within one, 62-61, with 54 seconds left in the third, but a layup by Aaron McKie off an nifty pass from Iverson made it 64-61 at the end of the quarter.
The Sixers led by as many 10 in the second quarter and held a 48-41 lead at the break. Iverson had 12 and Van Horn added 10 in the half.
Game notes
Portland's Scottie Pippen missed his second straight game with a left knee effusion and Jeff McInnis did not play due to a broken nose he suffered in Wednesday's win over Toronto. ... COVID-19病人的組織缺氧和呼吸衰竭（Hypoxia and lung failure in COVID-19):
在大部份的COVID-19的病人，組織缺氧（hypoxia)是COVID-19的病人主要的和最嚴重的症徵症狀。
維持足夠的氧氣供應釋出的補償機轉，包括
增加呼吸做功（increase respiratory effort)，
缺氧性的血管縮收（hypoxic vasocontriction)，以及
增加心輸出量（increase cardiac output)。
隨著COVID-19病人的嚴重度增加，這些補償的機制最後會失去它們的功效效力。
更進一步減少功能性的肺容積（functional lung capacity)，組織缺氧變得更嚴重且威脅生命，以及通常需要加護照顧的支持，造成COVID-19病人變嚴重的機轉，包括：
1.由於微血管的內皮細胞發炎炎症（endothelial inflammation)和微細小血栓（microthrombi)，以及肋膜積水（pleural effusion)，造成的增加死腔通氣（increased dead space ventilation)。
2.因為瀰漫性的肺泡發炎（alveolitis)和肺水腫（pulmonary edema)，造成氣體擴散的屏障升高（elevated diffuse barrier)。
3.因為肺塌陷（atelectasis)，肺實變化和纖維化（consolitation and fibrosis)，以及血管新生（angiogenesis)，造成由右向左分流增加（increased right-to left shunt)。
總合這些機轉，減少肺泡氣體交換的功能，因此造成嚴重的的動脈低血氧症（hypoxemia)和組織缺氧（hypoxia)。

<胎兒肋膜腔積液/胸腔積液/fetal pleural effusion>
最近COVID-19在台灣蠢蠢欲動，輕則完全沒症狀，重則引起發燒呼吸衰竭。
一般人喘起來，我們最常做的檢查就是照胸部X光，來看看有沒有肺炎或是肋膜積水；對於胎兒來說，我們則是利用照超音波的時候來檢查肺部有沒有異狀。

成人會肺積水，寶寶也會肺積水。
只是寶寶在肚子裡面有臍帶連接著，因此呼吸不是重點。嚴重的肺積水，不至於造成他們呼吸衰竭。
但是嚴重的肺積水，會造成肺部受到壓迫無法正常擴張，心臟血管受到壓迫影響回流，造成全身性的水腫，嚴重者可能胎死腹中😥。
有 #單純的肺積水，也有 #肺積水合併胎兒水腫。
會造成肺積水/胎兒水腫的原因很多，因此要一一做檢查來釐清。
1️⃣超音波檢查
🔎積水嚴重度: 判斷胎兒是只有單純肺積水，使否有壓迫到心臟，還是已經到了胎兒水腫的程度?
🔎從頭到尾仔細巡一遍: 先天性心臟病, 先天性肺部畸形 (常見的CPAM, 游離肺, 橫膈膜疝氣) 都有可能造成肺部積水，因此要注意是否其他器官的異常。
🔎羊水量: 另外可能會有羊水過多的情形，如果太多引起宮縮，有可能需要放水引流。
🔎血流阻力: 如果是胎兒貧血引起的水腫，有可能會引起中大腦動脈血流阻力過高，記得也要一併量測。
2️⃣染色體檢驗
以透納氏症, 唐氏症等較常見, 大概有10%會有染色體異常。
✏️因此會建議抽羊水做 #傳統染色體核型分析 和 #基因晶片 的檢驗。
3️⃣母血檢驗
🔎Rh血型不合
國人大多數為Rh陽性。若母親為Rh陰性，和寶寶不一樣，在肚子裡則有可能造成嚴重溶血反應。
🔎地中海型貧血
重症會造成嚴重胎兒水腫，甚至有可能到大週數才發生!
-->這些在第一次產檢的抽血都會檢驗
🔎先天性感染 (TORCH)
排除巨細胞病毒,弓漿蟲等先天性感染，這些有時候除了鈣化點或腦部異常，也可能造成胎兒水腫。
4️⃣如果排除以上異常，剩下的診斷大多數就是 #乳糜胸(Chylothorax)了
✏️追蹤方式
建議每兩週追蹤超音波，看是維持現狀還是變得更嚴重。
另外也會請孕婦回家多多注意寶寶的胎動!

通常單純的肋膜積水然後找不到其他原因，就會懷疑是下面這個病:
<#先天性乳糜胸/#乳糜管破裂>
一開始可能為單純的肋膜腔積液，後來進展變成回流受阻心衰竭，引發全身性的胎兒水腫。
在產前的胎兒治療，可以為單次的抽胸水，放置引流管，以及肋膜沾黏術。(其實就和成人的治療一樣)
✔️抽胸水
胸水再度累積起來的速度也很快，有可能需要多次的穿刺引流，間接造成破水的風險提高。
✔️放置引流管
可以持續引流胸水，缺點是有可能會被寶寶自己拔掉...
✔️肋膜沾黏術
就和成人打OK432這個藥物一樣。不過根據我們小兒科的研究數據統計，在出生後肺部纖維化比較嚴重一些，需要氧氣支持的機率較高。
✏️另外，依據之前和小兒科討論會的結果，如果能在出生前先幫胎兒引流胸水，對於預後有幫助。(引流的時間當然是越近越好囉)
